Evaluating the effectiveness of PCMHs

Dr. Ashwin Patel helps payers increase the effectiveness of their Patient-Centered Medical Homes (PCMHs). Using claims data, Dr. Patel helps payers understand the factors that drive member cooperation with the PCMH initiative, identify complex patients at the point of care, and determine the PCMH-based providers best able to reduce emergency room utilization.

Using claims data, Dr. Patel determines the PCMHs that are the most effective at achieving health plan objectives. These providers can then be evaluated more closely to facilitate the transfer of their best practices to more poorly performing providers. Furthermore, medically-homeless plan members can be directed towards the best performing PCMHs using the results of the analysis.
